How to Apply:

  1. If you are eligible (check here), the following documents are required to start the process:
  2. Customer Application
  3. Hold Harmless Agreement
  4. Utility Easement Deed – You must grant the County an easement for the DWTU and to allow operations and maintenance access.
  5. Tax Assessment for DWTS – After installation, ongoing operations and maintenance are provided by the County. The County assesses an annual fee for these services which is collected on your real estate tax bill ($55/month).
  6. Payment for the pro-rated share of operations is due upon unit commissioning (up to $660.00). You will be advised of the exact total upon completion of the Customer Application.

The Tax Assessment and Utility Easement signatures need to be witnessed by two individuals and notarized. Robert Rene at OnSyte is available to travel to your residence to offer assistance with both the notary process and the completion of the forms. Alternatively, if more convenient, the Public Works Department also has a notary available to help with the notary and completion of the forms.

Completed and executed forms can be mailed or delivered to the Public Works Department (350 N. Sinclair Avenue, Tavares, FL 32778-7800).
